How/Where do I obtain an ACH Debit Authorization form?

If you've provided a merchant your account and routing number to make payments, they should be able to withdraw funds without issue. Most of our accounts allow for ACH debits and credits at account opening. No form is required for this service.
Some businesses may require that you sign a form allowing them to withdraw payments from your account, but this form should be provided by that company and not by TD Bank.
If you are attempting to set up any kind of Direct Deposit, we do offer a Direct Deposit Form that may assist your employer in setting this up. This form can be accessed and printed from our website at https://go.td.com/1sSvhT6. It can also be obtained at any of our Stores.
