Based off of the information you have provided, it sounds like you are trying to enroll your personal account with another financial institution to a pay a TD Bank Credit Card automatically.
Fortunately we do have a form that can be downloaded online. Simply log on to your TD Credit Card account by visiting www.tdcardservices.com. If you have not enrolled your account, you can do so by selecting the "Set up your account now" link located on the right side of the page under "Need to enroll your Account?" or, you may click here to take you directly to the enrollment page: http://bit.ly/1mZCFef. There are three steps, and you will need your card number handy for the enrollment process.
Once you have logged in. click on "Make Payment" on the right hand side of the screen. After doing this you will see the "Download Auto-Pay Enrollment Form" on the right hand side corner. Then you will be able to fill out the form with the information you desire and mail it out to the address on the form. Presto! You have successfully enrolled for automatic payments.
