I need to have one completed by my local TD bank (for home equity line of credit), however my local branch told me to bring the form in for them to validate our ID and submit to your application dept. Many thanks!!!

We appreciate you visiting us on TD Helps today, Heather.
If you are US Resident you can obtain a CIP form (Consumer Lending ID Discrepancy Verification Form) at any local TD Bank, which is a standard form provided during the loan application process. For Non-Residents, we require a Customer Affidavit Form which can be emailed to you by contacting our Financial Solutions Group (FSG) 24/7 at 800-457-2387.
Hope this information help! Thanks for choosing TD Bank, have a good day.
