I need proof for the state that I no longer have an account with td bank

To assist you with this confirmation, we're happy to let you know that we are able to provide you with a Verification of Account. This request would apply to a Personal Deposit account you may have or had with us.
To provide this, we'll need a request in writing. When providing this documentation, please be as specific as possible with your request. Please also include where the completed request should be forwarded to (for example, does it need to be faxed, mailed or both). Also, be sure to include your authorization for us to release this information to your third party. Once the request is written including this information, kindly sign and date it, and have it notarized. It can then be faxed or mailed as below:
