On 2/1/2015 between 3 pm and 4 pm, I attempted to use the ATM machine at Xpect Discounts in Cromwell, CT. When I pushed the button for a $40.00 withdrawal from my TD Bank checking account. everything seemed to be working fine. There was a $1.50 fee, which I agreed to, and it said it was processing my request.
Next thing I know, the machine now says "WELCOME" and I did not receive my $40.00. I asked a store employee and was told the ATM was out of money and I needed to contact the number on the front of the ATM machine.
I called the number and was told to contact my bank.
Please let me know what form (s) I need to fill out so I can get this money back ASAP.
