Our Visa was compromised in early June. I have never rec'd the form to return to you. I have called several times and would like to get this settled. The fraudulent charges have been deleted but I need the form .Could you fax it to TD Bank in Brewer, ME 04412

If you have a TD Visa Credit Card and this is for a Credit Card dispute, you may fax paper work to 706-649-4073 if you have been advised to do so. You may also contact a TD Credit Card Specialist 24/7 at 888-561-8861 and speak with them directly for further assistance.

Justin, My problem is that I have not rec'd the form to fill out and mail back to TD Card Services.
TD Card Services issued a credit to our acct. But we still have not rec'd the form to fill out and mail back to you.
This has been going on for several weeks.

We appreciate you keeping us in the loop and we apologize to hear that you haven't received a copy of this form yet.
Typically, the Declaration of Fraudulent Activity form is mailed and you should receive it within 7-10 business days after the transactions in question have posted to your account. Since we understand that this time frame has passed and you haven't received it, we ask that you give one of our Specialists at TD Card Services a call. They're available for you 24/7 at 888-561-8861 to assist you with this form and the rest of the process.
