Thanks for dropping by TD Helps, Michelle. We'll be happy to provide the information that is needed to request a copy of your tax form.
TD Bank 1099-SA forms for HSA Distributions were generated and mailed by January 31, 2015 to our Customers. There is also a 5498-SA form that is for HSA Contributions that will be sent out in May, 2015.
If you have not received your 1099-SA form and wish to obtain the total amount of your distribution for the year of 2014 immediately, or would like to have a copy mailed to your address on file, you may feel free to give us a call, send a Secure Message via online banking or visit us at any TD Bank Store locations during normal lobby hours. You may choose the option that is most convenient for you!
