The Federal Trade Commission provides this page: http://1.usa.gov/1f6utCg which lists the steps you'll want to take to dispute this error. First, you'll want to report the incorrect info to the credit reporting companies and information on how to do this is given there.
The next step would be to report this error to us, TD Bank, if the credit card was reported as one of ours. You may review info to help with this process here: http://bit.ly/1riK35l.
