In section two of the Direct Deposit Authorization Form, you will need to first fill in your full name and address. Next the form asks for your telephone number. The next field asks for an employee ID which is typically used by your employer. If you do not have this information, you are able to leave this field blank or contact your employer for this information. Then you will need to fill in your Social Security Number. Be sure to fill out your account and routing numbers towards the bottom of the page.
