Hi Kristen, the form has some errors that I hope can be fixed immediately. The error is that when you review your entered information, its position appears differently than where you entered it. For example, your entered date of birth appears in the SIN number section (and there is no option to enter our last 4 SIN digits as requested by the form). The address line information ends up appearing in the City section, and so forth. Can you please forward this request to the team immediately so that the customers' information is recorded accurately? Thank you.
