Can the form be sent by mail since I am in TN and closest physical location is NC?
To set an account up, can it be done online and is there a minimum deposit required or can it be opened with the $10 from the Reading offer?

Thanks for joining the TD Helps community, Susan. We're happy to discuss this evening.
When opening one of our Young Savers accounts in the Summer Reading program, you would have to visit any TD Store location during lobby hours and you would be listed as the custodian on a minor's account. We apologize for any inconvenience this may cause. Feel free to also take a look at http://bit.ly/1pxm2Zo for more detailed information on this product type. For all nearby TD Store locations and their hours please visit http://bit.ly/TDStores.
Please keep in mind that you would need to provide your Social Security number, a valid form of identification to open the account, such as a State License, State ID or Passport. Acceptable forms of identification for minors include a School ID (preferably with a picture), Passport, US Birth Certificate or Social Security Card.
You're welcome to also take a look at our TD Bank Wow Zone. If you haven't done so already, please click here: http://bit.ly/1aT3ACT to take a look. You'll find a number of interactive games and features that you can share with kids. To learn more about our events and the summer reading program in particular, we recommend reaching our community contact through the "Contact Us" page mentioned in the link above.
We hope the information provided helps. Have a great upcoming weekend!
