Just noticed that my daughter's account is being hit with a $15/month "maintenance" fee. . What is the minimum balance she needs to keep in this account to avoid these fees?

Typically, a maintenance fee is charged when the account balance falls below the daily minimum balance at any time during the statement cycle.
