Our current Direct Deposit Authorization Form available via our website can be found at: http://bit.ly/1sSvhT6. Typically, this form is used with your employer or the Federal government to begin direct deposits of your payroll or benefits. Although this form doesn't specifically list pensions, we recommend contacting your depositor directly as they may be able to use it for this purpose.
You may also like to check with them to see if they have their own form, or if you may use a voided blank check or other means to establish your direct deposit with them. In many cases companies offer their own option that you can take advantage of for this purpose.
