TD Bank 1099-SA forms for HSA Distributions were generated and mailed by January 31, 2015 to our Customers. If you do not receive this form within 7-10 business days after January 31st, you will have a few ways to request a copy. You may give us a call, send a Secure Message or visit any TD Bank Store. You may to choose which option is most convenient for you! To connect with one of our Banking Specialists, you can call 888-751-9000, 24/7 and upon verification, they will be able to submit a request for a copy of your 1099-SA form. There is also a 5498-SA form that is for HSA Contributions that will be sent out in May.
To send a secure message you must first be enrolled in Online Banking and to start the process please visit here: http://bit.ly/1lU2qfY. Keep in mind that you will need your account number, ATM/Visa Debit Card number and email address. Once enrolled, you may send a secure message by selecting "Send a secure message" located on the left hand side of the page, beneath "Message Center." In your note, please be sure to include the account numbers, your current address and the reason you're requesting a copy (i.e. lost other copy, did not receive original).
